Scholarship Application Process |

CHS graduating seniors may apply for the Brad Kolo Memorial Excellence in Technology
Scholarship as follows:
- Submission of a
Student Application Form
- Submission of a CHS
Teacher Nomination Form
- Submission of a work sample, including
Work Sample
Teacher Validation Form
All scholarship application materials must be submitted to the
Collinsville High School Guidance office by May 15.

Scholarship Award and Recognition
|
|
|
Each scholarship recipient will receive $1,000
and have his/her name engraved on a
memorial plaque, which will be prominently displayed at Collinsville High School.
Recipients will be recognized at the May Collinsville Community Unit School District #10
Board of Education meeting and at the Collinsville High School Honors Convocation
ceremony. Scholarship recipients will be awarded $500 once proof of college enrollment and
fee payment has been received by the district. |

Selection |

Applications and portfolios for scholarship applicants will be reviewed by a team of
district administrators and staff as well as the Kolo family. Applicants will be notified
of their selection prior to the May school board meeting. |

What is the Brad Kolo Memorial
Excellence in Technology Scholarship?
In honor of Brad Kolo, a 1999 Collinsville High School graduate, a perpetual
scholarship fund has been set up in his memory to recognize CHS graduating seniors who
exemplify those characteristics that were indicative of Brad, as outlined below:
